Already booked through Zocdoc? Complete your registration below and select your final appointment time.

1. Choose your appointment time.
   Select your original Zocdoc date/time if available, or choose another available time. The time you book here becomes your actual appointment. The calendar displays Eastern Time.

2. Complete your registration.
   If you have not previously registered with Transform with Roman, select “No” when asked whether you are an existing patient.

3. Prepay to complete your booking.
   Your first appointment must be prepaid to complete the booking process.

Initial evaluation: $250, prepaid when booking.
Follow-up appointments: $250, automatically charged to your card on file at the time of each appointment.

All appointments are virtual. Our practice is fully self-pay and does not accept insurance. We accept major credit/debit cards and HSA/FSA cards. Superbills are available upon request; out-of-network reimbursement depends on your plan and is not guaranteed.
